Outcomes

Evaluation of outcomes and review of progress

We measure outcomes in order to provide feedback to all the individuals who use our services. Outcomes provide a way of assessing and monitoring an individuals progress while they are on their journey with us. They also help us all to focus on what the areas of need there are and as such they are used to drive the care pathway and care/support plans for each individual.

The types of outcomes we focus on include:

  • Improvement in health and emotional well-being
  • Increased Quality of life
  • Client satisfaction and making positive contributions
  • Increase in choice and control
  • Freedom from discrimination
  • Maintaining dignity and respect
  • Economic well-being
  • Greater community access and social inclusion
  • Reduction in restrictive practice
  • Reduction in need for paid staff

We use Audits from satisfaction surveys, complaints, incident reports, accident reports to name a few in order to help us track changes and progress. We also use standardised assessment tools.

We use evidenced based practice by utilising standardised assessment tools and outcome measures. Some of these include:

  • Health Equality Framework
